9e59d07947 Stop looping if all workers have died
If the workers are crashing and the restart limit has been met,
we need to stop listening for events and trigger an internal
error.

3799caa4fc Improve UsageError when used with --pdb
It might not be clear that pytest-xdist is used at all, and this should
help users to make --pdb work.
